Disc Jockey Questionnaire

1. Where do I send the completed Questionnaire?
• The Great Hall
• 1003 Perry Highway
• Pittsburgh, PA 15237
2. When is the completed Questionnaire due?
• Please mail it to The Great Hall about 10-14 days prior to the reception.
3. Should I enclose the final payment with the completed Questionnaire? 
• Yes, the final payment is due five days prior to the date of the reception.  
• The balance due (assuming a $100 deposit has been paid) is
  • $475 for wedding receptions in 2008 and 2009.
4. When will the Disc Jockey contact me?
• The Disc Jockey will be in touch with you after you have returned  the completed questionnaire to discuss your arrangements.
5. How can I reach the Disc Jockey?
• You can contact the Disc Jockey by calling The Great Hall at (412) 366-2610 and leaving a message.  Your call will be returned in a timely fashion.
6. Where can I obtain the Questionnaire?
• You can download the Questionnaire in Microsoft Word format by right-clicking the following icon and choosing "Save Target As" and then selecting the location on your computer to save the file.  This will allow you to complete the questionnaire using Microsoft Word.
